Quick answer · Community-reported standard

A 2 mg vial of LL-37 reconstituted with 1 mL of bacteriostatic water yields a concentration of 1.00 mg/mL. At that concentration, a 100 mcg dose of LL-37 equals 10 units on a U-100 insulin syringe (0.100 mL). LL-37 is typically administered twice daily via subcutaneous.

LL-37 2 mg reconstitution and dosage: 1 mL vs 2 mL bac water

For a 2 mg vial of LL-37, bacteriostatic water volume is flexible. Less water means higher concentration and smaller injection volumes; more water means lower concentration and easier dose precision. Both options below are valid for a 2 mg vial.

LL-37 reconstitution & storage

Reconstitute lyophilized LL-37 with bacteriostatic water. Add water slowly along the vial wall to avoid foaming. Allow to dissolve without shaking. Store reconstituted solution refrigerated.

Store lyophilized peptide at -20C for long-term storage. Reconstituted solution should be refrigerated at 2-8C and used within 2-3 weeks. Protect from light and repeated freeze-thaw cycles.

LL-37 dosing FAQ

How much bacteriostatic water should I add to a 2 mg vial of LL-37?

The community standard is 1 mL of bacteriostatic water for a 2 mg LL-37 vial. That gives a concentration of 1.00 mg/mL, so a 100 mcg dose is 10 units on a U-100 insulin syringe. Adding 1 mL instead doubles the concentration to 2.00 mg/mL and halves the draw to 5 units. Less water = smaller injection volume but harder to measure precisely.

How many syringe units is a 100 mcg dose of LL-37?

On a standard U-100 insulin syringe, a 100 mcg dose of LL-37 is 10 units when the 2 mg vial is reconstituted with 1 mL of bacteriostatic water (1.00 mg/mL). If you use 1 mL of bac water instead, draw 5 units (2.00 mg/mL).

What is the standard LL-37 dose?

Community-reported protocols for LL-37 typically use 100 mcg–200 mcg twice daily via Subcutaneous. 100 mcg is a common starting dose. Most commonly reported protocol; gradual titration from 50 mcg recommended

How long does a vial of LL-37 last at 100 mcg twice daily?

A 2 mg vial of LL-37 contains roughly 20 doses at 100 mcg per injection. At twice daily dosing, that is approximately 10 days (about 1 weeks) of supply per vial. Reconstituted LL-37 should generally be used within 28 days when refrigerated.

Where do you inject LL-37?

LL-37 is typically injected subcutaneous. Common sites reported in the literature and by users include: Subcutaneous injection in abdominal area, Subcutaneous injection in upper thigh. Rotate sites with each injection to reduce localized irritation.

Can I use sterile water instead of bacteriostatic water for LL-37?

Bacteriostatic water (containing 0.9% benzyl alcohol as a preservative) is strongly preferred for multi-dose vials like LL-37 because it inhibits microbial growth and lets a single vial be used over multiple weeks. Sterile water has no preservative and should generally be reserved for single-use preparations only.
