Oxytocin Dosing Calculator

Oxytocin: FDA-approved neuropeptide for labor and social bonding.

Pre-filled with the most-reported clinical reconstitution protocol for Oxytocin. See how 1 mL and 2 mL of bacteriostatic water change concentration and syringe-draw volume, then adjust the calculator below for your own vial.

Quick answer ยท Clinical protocol

A 5,000 IU vial of Oxytocin reconstituted with 2 mL of bacteriostatic water yields a concentration of 2,500 IU/mL. At that concentration, a 24 IU dose of Oxytocin equals 1 units on a U-100 insulin syringe (0.010 mL). Oxytocin is typically administered twice daily.

Oxytocin reconstitution: 1 mL vs 2 mL bac water

Bacteriostatic water volume is flexible. Less water means higher concentration and smaller injection volumes; more water means lower concentration and easier dose precision. Both options below are valid for a 5,000 IU vial.

BAC water addedConcentrationVolume per 24 IU doseDraw on U-100 syringe (units)
1 mL5,000 IU/mL0.005 mL0.5 units
2 mL2,500 IU/mL0.010 mL1 units
Based on a 5,000 IU vial of Oxytocin reconstituted with bacteriostatic water. Insulin-syringe units assume a standard U-100 syringe (100 units = 1 mL).

Alternative Oxytocin protocols

Beyond the default above, Oxytocin is also reported in the following protocols. Use these as starting points and recalculate with the tool above.

Standard Intranasal Social Protocol

Community
Dose
20-40 IU (international units)
Frequency
Once daily or as needed
Duration
Ongoing or situational use
Route
Intranasal

Most common community protocol; 24 IU is the dose used in most clinical trials; used before social situations

Low-Dose Daily Protocol

Community
Dose
10-20 IU
Frequency
Once daily
Duration
2-4 weeks
Route
Intranasal

Lower dose for regular daily use; some users report this is sufficient for anxiety reduction

Compounded Sublingual Protocol

Community
Dose
10-40 IU
Frequency
1-2 times daily
Duration
Ongoing
Route
Sublingual

Sublingual troches from compounding pharmacies; used by those with physician-prescribed protocols

Labor induction (FDA-approved)

Clinical
Dose
0.5-2 mU/min initial, increase by 1-2 mU/min every 30-60 min
Frequency
Continuous IV infusion
Duration
Until adequate labor established or delivery

Low-dose protocol. Maximum dose typically 20-40 mU/min. Requires continuous electronic fetal monitoring and trained obstetric personnel. Controlled infusion pump mandatory.

Labor augmentation (FDA-approved)

Clinical
Dose
0.5-2 mU/min initial, titrate to contraction pattern
Frequency
Continuous IV infusion
Duration
Until adequate labor pattern achieved

Used when spontaneous labor is inadequate. Lower doses often sufficient compared to induction. Same monitoring requirements as induction.

Oxytocin reconstitution & storage

Pharmaceutical-grade oxytocin (Pitocin/Syntocinon) is provided as a ready-to-use sterile solution for injection (10 units/mL). For IV infusion, dilute in normal saline or lactated Ringer's solution. For intranasal research use, oxytocin is provided in metered-dose nasal spray devices (typically 4 IU per spray). No reconstitution required for commercial formulations.

Store injectable oxytocin at 2-8C (refrigerator). Protect from light. Do not freeze. Intranasal formulations: store at room temperature (15-25C) or as specified by manufacturer. Discard per manufacturer expiration date. Multi-dose injectable vials should be discarded 28 days after opening.

Oxytocin dosing FAQ

How much bacteriostatic water should I add to a 5,000 IU vial of Oxytocin?

The community standard is 2 mL of bacteriostatic water for a 5,000 IU Oxytocin vial. That gives a concentration of 2,500 IU/mL, so a 24 IU dose is 1 units on a U-100 insulin syringe. Adding 1 mL instead doubles the concentration to 5,000 IU/mL and halves the draw to 0.5 units. Less water = smaller injection volume but harder to measure precisely.

How many syringe units is a 24 IU dose of Oxytocin?

On a standard U-100 insulin syringe, a 24 IU dose of Oxytocin is 1 units when the 5,000 IU vial is reconstituted with 2 mL of bacteriostatic water (2,500 IU/mL). If you use 1 mL of bac water instead, draw 0.5 units (5,000 IU/mL).

What is the standard Oxytocin dose?

Clinical protocols for Oxytocin typically use 24โ€“40 IU twice daily via subcutaneous injection. 24 IU is a common starting dose. Standard research dose in adults. NEJM ASD trial used age-adjusted doses (8-40 IU). Administered via nasal spray device. Effects may be context-dependent and individually variable.

How long does a vial of Oxytocin last at 24 IU twice daily?

A 5,000 IU vial of Oxytocin contains roughly 208 doses at 24 IU per injection. At twice daily dosing, that is approximately 104 days (about 15 weeks) of supply per vial. Reconstituted Oxytocin should generally be used within 28 days when refrigerated.

Where do you inject Oxytocin?

Oxytocin is typically injected subcutaneously. Common sites reported in the literature and by users include: Intravenous (continuous infusion via controlled pump, for obstetric use), Intramuscular (deltoid or gluteal, for PPH prevention), Intranasal (nasal spray, for research applications). Rotate sites with each injection to reduce localized irritation.

Can I use sterile water instead of bacteriostatic water for Oxytocin?

Bacteriostatic water (containing 0.9% benzyl alcohol as a preservative) is strongly preferred for multi-dose vials like Oxytocin because it inhibits microbial growth and lets a single vial be used over multiple weeks. Sterile water has no preservative and should generally be reserved for single-use preparations only.
